This is done with your active permission and participation. The personal information you provide will be used to customize your case file and facilitate communication and collaboration.

If you request information via email, you will be asked for your name, email address and telephone number in order to respond to your request and/or to provide additional information pertinent to your case and other services.

            Non-Personally Identifiable Information: This refers to information that does not by itself identify a specific individual.
